How are you? Questions of this nature are non committal, it’s so open ended that the person responding is faced with the decision about the level of detail, length, and guess as to the intention. Don’t get me wrong, it’s a standard polite greeting, i use it all the time, but the only purpose for me is finding out whether or not someone wants to talk or not.
If you really want to know how someone is doing, be more direct. How did your meeting go? Did you have your first coffee yet? Making a question personal makes it feel more genuine and you are more likely to get a well thought out response. Adding a relevant topic in your question takes the guess work away, and a simple yes or no answer is usually all you need if your questions are direct.
